If you are paying by Credit Card and the Card Holder Name is different from the Registered Delegate Name, you are required to provide a Credit Card Authorization form completed by the Card Holder together with a copy of the front and back of the Credit Card. *Students/Trainees - MUST state the name of the Institution of where they are studying or working on their Registration Form. As well, students and trainees are required to submit an official letter on their Institution’s letterhead OR photocopy of their 2009/2010 Student/Trainee ID from the Institution where they are studying indicating proof of their Student/Trainee status. Please fax the proof of Student/Trainee status or mail to the Conference Secretariat. Registration will not be processed without receipt of this documentation.

**The Student/Trainee registration fee is subsidized by the Congress, however, the Gala Evening is not.

The Cultural Evening is not included in registration, but tickets may be purchased by registrants and accompanying persons for $65.00 CAD each.
The Gala Evening is included in Member, Non-Member and Accompanying Person registration, but is not included in Student/Trainee registration unless the Student/Trainee registration including the Gala Evening is purchased.

HOW to MAKE PAYMENT

Full payment of registration fees must be received by each deadline in order to qualify for the stated fees.

Payment can be made by Company Cheque, Visa, MasterCard, AMEX, Money Order, Bank Draft or Bank Transfer.

Charges on your credit card statement will appear as “International Conference Services” and will be converted to your currency.
Please ensure that your name and the words “EVENT: TTS2010” appear clearly on money orders, bank drafts, bank transfers or company cheque payments.
Make company cheques payable to “International Conference Services c/o TTS2010”.

Bank drafts, money orders and company cheques will only be accepted in CAD dollars and must be drawn on a Canadian Bank.
It is the Delegate’s responsibility to ensure all bank transfer fees are paid over and above the registration fees owed. Otherwise, delegates will be asked to pay any outstanding balance at the on-site registration desk.

Please note all prices are in CAD dollars. Payments made over the internet are via a secured site. For registration inquiries, please send an email to: tts2010-registration@meet-ics.com. Please keep a photocopy of the Registration Form for your own reference and bring it with you to Vancouver, Canada.

REGISTRATION CONFIRMATION
All registrations will be acknowledged in writing with confirmation of the Delegate’s requirements according to the submitted Registration Form. The Registration Form acts as a Tax Invoice.

Includes all applicable taxes.

REGISTRATION CANCELLATION & REFUND POLICY
Registration cancellations received via email or mailed to the TTS2010 Secretariat’s address by Friday 25 June 2010 will be accepted and all fees refunded, less a $150 CAD administrative fee (as per Committee policy). Cancellations received after Friday 25 June 2010 will not be refunded; however, transfer of your registration to another person is acceptable. A completed Registration Form for the new person must be emailed, faxed or mailed to the Congress Secretariat prior to the Congress, explaining who is being replaced. DO NOT USE THE ON-LINE REGISTRATION FORM FOR THIS PROCEDURE. No refunds will be made for nonattendance at the Congress.

In the unlikely event that TTS 2010 is cancelled, travel and accommodation costs will not be refunded.
